Please email insuranceeligibility@headspace.com with the name of your current insurance company and ID number, along with a copy of the front and back of your member ID card. If you're not the plan subscriber, please provide the name and date of birth of the individual who is. Please note that any delays in providing insurance information may result in your appointments being billed to you at our self-pay rates.
