If your company offers Headspace EAP, you have several convenient ways to request in-person therapy:

Through the Headspace App:
- Open the Headspace app.
- Navigate to the "Care" tab.
- Select the "In-person care" tile.
- Follow the prompts to request a match with a provider.

Requesting through your Coach or Clinician (within the app):
- If you're already working with a Headspace coach or clinician, simply ask them about in-person care.
- Fill out a form with a couple details related to the care you seek.
- Our Care Concierge team will then share provider options and availability details with you via in-app chat or email.

Through the Headspace Hub (Self-Service Online Form):
- For direct access to in-person therapy without engaging with a coach or the full app experience, visit the Headspace Hub.
- Go to: https://my.headspace.com/hub (or use the direct link provided by your employer if it's different).
- From the Headspace Hub, select "In-person therapy" and fill out the self-service request form.
