What Is a Release of Information and How Can I Request One?
A Release of Information (ROI) allows you to authorize the release of your personal health information (PHI) to yourself, a legal representative, or an outside party (e.g., employer, provider, or insurance company). This process helps protect your privacy and ensure compliance with HIPAA regulations and the Headspace Privacy Policy.

What Can Be Requested Through an ROI?
Common requests include:
Treatment summaries
Letters confirming diagnosis or clinical care participation
Forms for Leave of Absence, FMLA, or workplace accommodations
Clinical session notes or medical records
Verbal coordination of care with off-platform providers or insurers
Please note: Headspace cannot release medical records under court mandate at this time.
Requirements Before Submitting a Request
To fulfill any ROI-related request, members must first establish an ongoing clinical relationship with a Headspace provider. This typically includes:
An initial intake appointment
Additional follow-up sessions to assess care needs
The number of sessions required may vary. Completing sessions does not guarantee that a provider can complete specific documentation or forms. Final decisions are made at the discretion of the treating provider.
If you're already enrolled in clinical care, we recommend discussing your request directly with your provider.
How to Submit a Release of Information Request
Once you’ve spoken with your provider:
Email us at carerecords@headspace.com

Include the following:
Your email on file with Headspace
The type of request (e.g., treatment summary, FMLA paperwork)
The name of the party you’d like the information released to
After your request is reviewed and the ROI form is completed, it will be forwarded to your provider(s) for clinical review. Most requests are processed within 30 days.
