A: Roughly every 90 days, you'll retake the short questionnaire (PHQ-9/GAD-7) so we can update your care plan. Every check-in is reviewed for signs you might need extra support:
- If we see some patterns of concern, we'll send an update to your PCP (if you've consented) — you'll stay enrolled in the program.
- If a check-in indicates you may be at risk of harming yourself, you'll immediately see in-app crisis resources (including the 988 Suicide & Crisis Lifeline), and we'll notify your doctor. In this case, you'll be moved out of the program so you can get more intensive support.
Q: What happens if I don't complete my check-in?
A: If we're unable to reach you to complete your check-in questionnaire after 3 or more outreach attempts over a 30-day period, you may be disenrolled from the program for loss of contact.
